Bowtie Advanced Imaging Limited-time Offer ("the Offer") is only applicable to advanced imaging scan which is performed in designated imaging center arranged by Bowtie Health Specialist upon obtaining a claim pre-approval between the promotion period from Jun 24, 2024 to Sep 30, 2024 (”Promotion Period”). This advanced imaging has to be pre-approved for “Cashless Settlement ”.The health specialist will apply for pre-approval from Bowtie on your behalf and you are not required to fill out the pre-approval application yourself. Once your application is approved, you can enjoy the “Cashless Settlement 2”.
Terms and conditions of Special Rewards
Each Bowtie VHIS insured customer arranged by Bowtie Health Specialist to complete an advanced imaging scan in the designated imaging center upon obtaining a claim pre-approval are entitled to receive the below supermarket cash voucher special rewards.

To receive special rewards, you need to meet the following conditions: a. Your submitted claims in the policy year has not reached the the item maximum limit of Prescribed diagnostic imaging tests under basic benefit or the policy annual benefit maximum limit; b. You get a referral letter from your doctor (if you do not have a valid referral letter, Bowtie Health Specialist will arrange a doctor consultation with Bowtie & JP Health’s doctor and recommendation will be given by doctor if it is medical necessary for the imaging scan); c. Your advance imaging scan is medically necessary; d. Your advanced imaging scan must be performed in the designated lab imaging center arranged by Bowtie Health Specialist upon pre-approval. You will enjoy the “Cashless Settlement 2”. Customer is required to submit the Imaging report to Bowtie for final claim assessment. You are entitled to receive the special rewards if the claim is approved and completed. Bowtie will deliver the special rewards within 45 days from the day of imaging scan performed.
If your VHIS policy has a deductible amount per policy year (ie Bowtie Pink) and your accumulated eligible medical expense has not reached the deductible amount, pre-authorization will not be approved and special rewards will not be applicable.
All claims settlement will be subjected to the final bill, the policy terms & conditions and the exclusions set out in the policy terms & conditions. If the final bill includes excluded items under the Insured’s policies, special rewards will not be applicable.
Bowtie reserves the right to make changes to the reward offer or to amend terms and conditions related to the reward offer at any time without prior notice.
Terms and Condition of Pre-approval Claims
The “Cashless Settlement” service is not a contractual service but an administrative arrangement offered in our absolute discretion in respect of covered expenses incurred by the Insured during the treatment/ hospitalization. It is subject to termination by Bowtie at any time without prior notice.
If the diagnostic imaging, treatment or hospitalization is due to illness or disability classified under any exclusion or whatsoever of the concerned policies, claim will not be approved.
Obtaining a pre-approval shall not be deemed as an agreement to pay for any payment on behalf of the Insured, all claims settlement will be subjected to the final bill, the policy terms & conditions and the exclusions set out in the policy terms & conditions. If the final bill includes excluded items under the Insured’s policies, the Insured shall pay for such expenses upon completion of diagnostic imaging scan.
If there is co-insurance for the relevant benefit item in the policy, the customer needs to pay the co-insurance medical expenses on the day of the imaging test.
If the pre-approval is not approved, Bowtie will notify the insured and will not arrange an appointment for imaging testing. Insured person must arrange the imaging test themselves if necessary.Customers can also submit claims for further review and after completing the imaging test.
Insured person shall bear all costs incurred for bills which are not medically-related or when the bill exceeds the given limit. Unless pre-approval, any personal expenses will be on the insured's own account.
If the final bill exceeds the Insured’s eligible benefit limit under his/her policy(ies) we also reserve the right to claw back the amount from you.

In case of Tropical Cyclone Warning Signal No.8 or above or Black Rainstorm Warning Signal is hoisted at the time of advanced imaging, the designated imaging center will be closed and the advanced imaging will be rescheduled.
Insured person who needs to cancel the appointment must contact us before 12 noon one working day in advance of the original scheduled date. Failure to notify the cancellation within the specified time or failure to show up on appointment day without prior notification will be charged for the cost of glucose tracer which has been pre-ordered. The price ranges from HKD$2500 to HKD$8000. Bowtie will charge you relevant fees.
Bowtie will collect your personal information (such as name, phone number, medical records) for pre-approval and appointment purposes and may disclose this information to Bowtie & JP Health and the designated imaging test center for the appointment arrangement.
